Annual Title Agent/Agency Review - Electronic Filing Form

The Annual Title Agent/Agency Review-Electronic Filing Form Application allows Ohio title agents and agencies to electronically file their Annual Title Agent/Agency Review Form with the Ohio Department of Insurance. 

Who Should Use This Application

Only licensed Ohio title agents and agencies or their authorized representative required to file an Annual Title Agent/Agency Review Form.
